As a complement to a low-calorie diet and increased physical activity for long-term use to correct body weight in adult patients with BMI ≥30 kg/m2 (obesity) or ≥27 kg/m2 and <30 kg/m2 (overweight) in the presence of at least one associated associated disease/condition (such as pre-diabetes), diabetes mellitus type 2, arterial hypertension, dyslipidemia or obstructive sleep apnea syndrome).
Saxenda® is injected once a day at any time, regardless of food intake. It should be injected into the abdomen, thigh or shoulder area. The place and time of injection can be changed without dose correction. However, it is recommended to give injections at about the same time of day after choosing the most convenient time.
Doses
The initial dose is 0.6 mg per day. The dose is increased to 3 mg per day, adding 0.6 mg at intervals of at least one week to improve gastrointestinal tolerance. If the new dose is not well tolerated for 2 consecutive weeks when the dose is increased, consideration should be given to discontinuing therapy. Use of the drug in a daily dose exceeding 3 mg is not recommended.
The therapy with Saxenda® should be discontinued if after 12 weeks of using the preparation in a dose of 3 mg per day the loss in body weight was less than 5% of the initial value. The need to continue therapy should be reviewed annually.
Missed dose
If less than 12 hours have elapsed after the normal dose time, the patient should administer the dose as soon as possible. If less than 12 hours have elapsed before the normal time of the next dose, the patient should not give a missed dose but should resume injection at the next scheduled dose.
No additional or increased dose should be given to compensate for a missed dose.
Patients with diabetes mellitus type 2
Saxenda® should not be used in combination with other EPP-E receptor agonists.
At the beginning of therapy with Saxenda® it is recommended to reduce the doses of simultaneously applied insulin secretogens (such as sulfonyl-urea preparations) to reduce the risk of hypoglycemia.
Elderly patients (≥ 65 years)
No age-related dose adjustments are required. Experience with the drug in patients ≥ 75 years of age is limited, use of the drug in these patients should be with caution.
Patients with renal failure
In patients with mild to moderate renal impairment (creatinine clearance ≥ 30 ml/min) no dose correction is required.
There is limited experience with Saxenda® in patients with severe renal impairment (creatinine clearance ≥ 30 ml/min). The use of Saxenda® in such patients, including patients with terminal renal failure, is contraindicated.
Patients with liver dysfunction
For patients with mild to moderate liver dysfunction, use with caution.
The use of Saxenda® in patients with severe liver dysfunction is contraindicated.
Children
The use of Saxenda® for children and adolescents under 18 is contraindicated due to the lack of data on safety and efficiency.

Experience of using Saxenda® in patients with inflammatory bowel disease and diabetic paresis of the stomach is limited. Lyraglutide use for such patients is not recommended as it is associated with transient undesirable reactions from the gastrointestinal tract, including nausea, vomiting and diarrhea.
Be careful:
Patients with chronic heart failure class I-II according to the NYHA classification, mild to moderate liver function disturbance, pancreatitis in history, thyroid disease; patients ≥ 75 years old.
Patients reported that overdoses were accompanied by severe nausea, vomiting and diarrhoea, but in all cases the condition of patients recovered without complications. There was no severe hypoglycemia in any of the cases.
In case of overdose, appropriate symptomatic therapy should be performed.
GPP-1 is an intestinal hormone that is actively released after a meal. It is responsible for a sense of satiety, reducing the feeling of hunger, and thus the regulation of appetite. In addition, GPP-1 stimulates the release of insulin and inhibition of glucagon synthesis corresponding to blood glucose concentration.
Saxenda® is an analogue of glucagone-like peptide-1 (GPP-1), 97% identical to the natural human GPP-1. Saxenda® is the first of its kind to be registered for use in obesity therapy in the USA, Canada, EU and Russia on the basis of GPP-1. Today the product is available in more than 45 countries.
The drug is injected subcutaneously using a handy multi-dose automatic syringe handle. The active substance, entering the blood, activates GPP-1 receptors in the brain and causes a feeling of dryness. Thus, taking Saxenda® once a day reduces appetite and, consequently, the amount of food consumed.
Saxenda ® not only reduces the waist circumference, but also the volume of visceral fat, which is important for secondary prevention of complications. Thus, in addition to its direct action, the drug reduces blood pressure figures in hypertension, reduces blood cholesterol levels, facilitates night apnea symptoms.
The efficacy and safety of Saxenda® has been carefully studied in extensive clinical trials.
Studies involving 5,000 patients observed between 6 months and three years showed that approximately 90% of patients who took the drug regularly resulted in weight loss. 56 weeks of medication combined with diet and exercise resulted in a 9.2% weight loss.
Significant results of the study:
- Weight loss of 5% or more in 63 percent of patients treated with Saxenda® versus 27 percent in the placebo group.
- Weight loss by 10% or more in 33% of patients in the group on therapy with Saxenda® versus 11% in the placebo group.
- Reduction of blood glucose level at initially elevated figures (prediabetes) in obese patients against the background of Saxenda®.
- Saxenda® was well tolerated. Slight side effects of the gastrointestinal tract were transient and disappeared within 4 weeks
