Apparently, Ursofalk has a slight inhibitory effect on the synthesis and secretion of endogenous bile acids into bile and, apparently, does not affect the secretion of phospholipids into bile. With regular use, the concentration of UDCA in bile reaches an equilibrium state after about 3 weeks.

UDCA solubilizes cholesterol and is incorporated into micelles. It also acts, presumably, by a unique mechanism, causing the dispersion of cholesterol in the form of liquid crystals in an aqueous medium. As a consequence, even with the introduction of high doses (for example, 15–18 mg / kg / day), a concentration of UDCA above 60% of the total amount of bile acids is not achieved. Bile, rich in UDCA, effectively solubilizes cholesterol. The overall effect of UDCA is to increase cholesterol levels. The different mechanisms of action of UDCA combine to alter the state of bile in patients with gallstones, which helps dissolve cholesterol stones. After stopping the administration of UDCA, the concentration of this bile acid in bile falls exponentially, decreasing to approximately 5–10% of its steady state level in about 1 week.

Other mechanisms of action include cytoprotection of the epithelium of cells of the damaged bile duct (cholangiocytes) from the toxic effect of bile acids, inhibition of hepatocyte apoptosis, immunomodulatory action and stimulation of bile secretion by hepatocytes and cholangiocytes.
